  • Your Unique Student Identifier Number.

  • If you are applying to enrol into a CHC Community Service qualification, you will be required to upload a police check, unless you are working in a related industry.

  • If you are enrolling into a full qualification, you will be required to complete a Language, Literacy and Numeracy (LLN) test. This will be sent to you electronically on completion of this application. Please note that you will not be enrolled into your chosen qualification until the LLN test has been satisfactory completed.

Privacy and your personal information

Under the Data Provision Requirements 2012, Sureway Skills Training is required to collect personal information about you and to disclose that personal information to the National Centre for Vocational Education Research Ltd (NCVER).Your personal information (including the personal information contained on this enrolment form), may be used or disclosed by Sureway Skills Training for statistical, administrative, regulatory and research purposes. Sureway Skills Training may disclose your personal information for these purposes to:

  • Commonwealth and State or Territory government departments and authorised agencies as well as your Workforce Australia services or Disability provider (if applicable); and

  • populating authenticated VET transcripts;
